Birth Certificates

A birth certificate is a confidential document, and only the individual, biological or adoptive parents, biological or adoptive grandparents, legal guardian, adult child, or spouse can apply for the birth certificate. To obtain a birth certificate you must present a valid, government issued picture I.D. (driver's license, military I.D., state issued I.D.).

Fees - Certified Birth Certificates

  • $10.00 for the first certified copy and $5.00 for each additional certified copy - PER PERSON.
  • In order to receive the discount on additional birth certificates, they must be requested at the same time as the original request. Otherwise, each time you request a birth certificate it will be $10.00 for the first certified copy and $5.00 for each additional certified copy, per person.

Payment can be in the form of cash, personal check, or money order (check or money order should be made payable to Probate Court). No counter checks accepted.

Hours

  • 8:00 a.m. to 4:30 p.m. Monday through Friday. Please check for holiday closings.

If you were born in the state of Georgia, you can now obtain your certified birth certificate in any county Vital Records office or Health Department in Georgia. If you would like to obtain your birth certificate from Gwinnett County, you can come in person or send in a written request to:

If you are requesting your birth certificate through the mail you need to include the following information: Full name of the child, mother's maiden name, father's name, and child's date of birth. You must include your check or money order made payable to Probate Court, a self-addressed stamped envelope back to yourself, and a clear photo copy of your picture I.D. You can also obtain your birth certificate from the:

When requesting your birth certificate from the Department of Human Resources, payment can be made in the form of money order only.

Born outside Georgia - If you were born outside the state of Georgia, you will need to contact the county or state department where the person was born. Or, you can call Vitalchek at 1-800-255-2414 or www.vitalchek.com.
