Death Certificates

You may obtain a death certificate by phone, mail, or in person from the Gwinnett County Probate Court.

Fees - Certified Death Certificate

  • $10.00 for the first and $5.00 for each additional certified copy – PER PERSON
  • In order to receive the discount on additional death certificates, they must be requested at the same time as the original request. Otherwise, each time you request a death certificate it will be $10.00 for the first certified copy and $5.00 for each additional certified copy, per person.
  • Cash, Personal Check or Money Order (check made out to Probate Court)
Hours
  • 8:00am – 4:30pm / Monday – Friday
Information required to receive a death certificate:
  • Name of the deceased
  • Date of death
  • County of death
  • County of residence

Requests can be made in person or through the mail. Submit request to:

When ordering a death certificate through the mail please include the following information: Full name of deceased, date of death, county of death, and county of residence. Also include a check or money order and a self-addressed, stamped envelope.
