You may obtain a death certificate by phone, mail, or in person from the Gwinnett
County Probate Court.
Fees - Certified Death Certificate
- $10.00 for the first and $5.00 for each additional certified copy – PER PERSON
- In order to receive the discount on additional death certificates, they must be requested at the same time as the original request. Otherwise, each time you request a death certificate it will be $10.00 for the first certified copy and $5.00 for each additional certified copy, per person.
- Cash, Personal Check or Money Order (check made out to Probate Court)

Information required to receive a death certificate:
- Name of the deceased
- Date of death
- County of death
- County of residence

When ordering a death certificate through the mail please include the following
information: Full name of deceased, date of death, county of death, and county
of residence. Also include a check or money order and a self-addressed, stamped
envelope.
