Please use the following forms to complete an application, then print and sign the form before mailing it to the Gwinnett County Elections office.

(3) After all required fields are completed, click on the "NEXT" button to proceed to the printable form.

To register to vote in Gwinnett County, Georgia

Note: If you are submitting this voter registration form by mail and you are registering for the first time in Georgia, attach a copy of one of the following items with your application: a copy of a current and valid photo ID or a copy of a current utility bill, bank statement, government check, paycheck, or other government document that shows your name and address. Those who are entitled to vote by absentee ballot under the Uniformed and Overseas Absentee Voting Act are exempt from this requirement.

If you think you are registered but unsure of your status, please visit the Georgia Secretary of State's web site. Here you can check your official registration record to determine if you need to either re-register or just make a change.

If you are registering to vote for the first time, fill out the form (see below), complete it in its entirety, print and enclose one of the approved identifications listed above.

If you need to change your name and/or address, fill out the form (see below), and complete all blocks on the form, print and mail. Although you are registered to vote the form is an official government document and therefore, MUST be completed fully.

| Step 4: |
In approximately 4-6 weeks you should receive a precinct identification card that will list your political districts and polling location. If you are unsure of your poll, visit the Georgia Secretary of State's web site. You will be shown a map of your area and where your poll is located. |
