All forms are in PDF format and will require the
Adobe Acrobat Reader 7.0 or higher. From our website, right click on the form name. At the menu, select "Save Target As". Save the form to a location on your PC. Open the selected form in Adobe Reader, click on Tools, Typewriter. Please note - when checking boxes on the forms, check all appropriate boxes on the form prior to clicking Tools, Typewriter.
