Reasonable Modification Request Form

Gwinnett County is committed to compliance with Title II of the Americans with Disabilities Act and provides reasonable modifications to ensure its residents have equal access to all county-sponsored programs, services, and activities and does not discriminate on the basis of disability in the County's programs, services, or activities.

This form may be used to request a reasonable modification or auxiliary aid or service that may be needed by an individual to access County programs, services, or facilities operated or provided under the authority of the Gwinnett County Board of Commissioners. Please submit this form no later than two business days before the event in which an accessibility modification is needed.
