Employee Wellness Center Benefits 

In-person and online services at the Gwinnett Employee Wellness Center: 

  • Primary care services for sick care, minor injuries, preventive screenings, immunizations, and managing chronic conditions like diabetes or high blood pressure

  • Health coaching to help you live a healthy lifestyle, develop sustainable habits, and improve your sleep, stress, diet, and/or fitness level

  • Select pediatric care for sick care and sports physicals

  • Care navigation to book nearby, in-person appointments or tests you may need outside of Crossover and to manage medical records and prescription orders (virtual only) 

  • Occupational medicine for pre-employment, fit-for-duty, and return-to-work exams, drug testing, and work-related vaccines

General

No. The name of the wellness center will remain the Gwinnett Employee Wellness Center.

No. The hours of operation will remain the same:

  • Monday, Wednesday, and Friday from 7:00am to 4:00pm; closed for lunch from 11:00am to noon
     
  • Tuesday and Thursday from 10:00am to 7:00pm; closed for lunch from 2:00pm to 3:00pm

Crossover works with you to develop an ongoing relationship with an interdisciplinary care team that will get to know you and your health needs so you can trust them with your care. The care team consists of a family medicine physician and nurse practitioner, a health coach who is a registered dietitian, and care navigator, all of whom work together to provide holistic, comprehensive care. You can learn more about the providers on the Crossover Platform under the option, “Meet Your Care Team.”

No. All eligible employees will need to register a new account with Crossover Health — it’s fast and easy!

  • Employees and their adult dependents ages 18 and child dependents ages 3 to 17 enrolled in an eligible Aetna or Kaiser health plan

  • Pre-Medicare retirees and eligible dependents enrolled in an eligible Aetna health plan

  • Primary care: Get an annual exam, sick care, preventive screenings, request labs, and order prescriptions. Plus, get help managing long-term conditions like diabetes and high blood pressure.  

  • Health coaching: Work with a registered dietitian who can help with nutrition, fitness, sleep issues, and setting/achieving your goals for a healthy lifestyle. Crossover’s Health Coaching also includes support for managing weight, improving work-life balance, and quitting smoking.  

  • Select pediatric care: Get annual exams, sick care, and sports physicals. 

  • Care navigation: A virtual care navigator will be your guide from start to finish for any services or appointments you need outside of Crossover.  

  • Prescription orders and refills: Prescriptions can be sent straight to your local pharmacy. Some medications may be able to be filled at the health center by your provider.

  • Child dependents are eligible to receive sick care and sports physicals
     
  • Eligible Aetna and Kaiser members ages 18 and older are eligible to receive health coaching with a registered dietitian

Crossover Health provides advanced primary care via a collaborative, holistic, flexible approach that brings primary care, health coaching with a registered dietitian, pediatrics, and care navigation together to deliver better experiences and industry-leading outcomes. Available through in-person and/or online care channels, Crossover allows members to develop an ongoing, trusted relationship with their dedicated, connected care team. Crossover’s providers meet members where they are on their health journeys, and address members’ health history, environment, and the unique circumstances that affect their overall well-being. Providers and members work together to identify, set, and achieve health and wellness goals. If a member needs care outside of Crossover, they help every step of the way. Crossover sees health as it should be — connected and without barriers.

Crossover’s flexible approach makes it easy for you to get quality care and connect with their passionate team of providers through multi-channel access: online via messaging and video visits, over the phone, or through in-person appointments at the Gwinnett Employee Wellness Center.

Start by signing up for a Crossover account. Once you become a Crossover member, you get access to Crossover’s team of providers at the Gwinnett Employee Wellness Center, including a family medicine physician, medical assistant, health coach who is a registered dietitian, and care navigator. There are no complicated bills, insurance submissions, or unnecessary paperwork. The care you receive from Crossover will be free to low cost.

Crossover’s dedicated teams of healthcare professionals deliver comprehensive, coordinated care — in person and virtually at the Gwinnett Employee Wellness Center. Crossover’s secure technology connects you to the team, and the team to each other. The team includes a board-certified family medicine physician, nurse practitioner, health coach who is a registered dietitian, and care navigator.

We understand that everything is connected to your overall well-being. With the Crossover team, you see the same care team every time. Your providers work together to get to know you and meet your health needs. By using a collaborative team of providers across clinical disciplines, the care team helps close care gaps that siloed, single-specialty providers may miss.

All your providers work together to help you get and stay healthy, and you see the same care team every time you need it, whether in-person or virtually. You can send messages to your care team and they can answer your questions, all in the Crossover Platform or app. In addition, the Crossover Care Team includes a care navigator, who will personally guide you when you need to access care outside of Crossover. The care navigator will also help arrange tests and lab work, and make sure your records are transferred back to Crossover. 

Your Crossover Care Team can provide you or your community-based provider with a copy of your Crossover medical records if the appropriate consent documents are signed. You can limit the amount and types of information that you share (e.g., medication list, medical bills, lab results, consultation results, etc.). Crossover can also receive any medical records from your community-based providers.

The Crossover Care Navigator will help coordinate referrals to care specialists outside of Crossover, including referrals to surgery, imaging, and other specialty services. The care navigator will help identify high-quality, in-network specialists, assist with prior authorizations, and help close the loop with the referring primary care provider.

Yes, you can use Crossover providers for some services and other providers for other services depending on your health needs.

Yes, you can use Crossover's provider as your primary care provider. This may consist of routine preventive exams in addition to chronic condition management. If you do not wish to use a Crossover provider as your primary care provider, you may still receive health coaching with a registered dietitian at the Gwinnett Employee Wellness Center for other health needs.

Yes. The choice is yours. You are not required to switch your primary doctor to use Crossover services.  At your request, your records from your Crossover Care Team can be sent to your community healthcare provider or vice versa. 

Your Crossover Health Coach, as a registered dietitian, is equipped with the expertise to guide you toward optimal health and wellness through personalized nutrition plans and lifestyle adjustments. They view food as a fundamental component of your overall well-being and understand its potential to serve as medicine in supporting your health goals.

Whether you're aiming for weight management, seeking sports nutrition advice, looking for meal-planning guidance, or navigating specific health concerns or diagnoses, your Crossover Health Coach is your advocate. They're committed to understanding your unique needs and empowering you to make sustainable changes by focusing on behavior modification, motivation, and holding you accountable along the way.

Beyond nutrition, your health coach is prepared to assist you in managing exercise routines, stress levels, sleep patterns, and other lifestyle habits that contribute to your overall health. With their guidance and support, you can be put on the path to achieve lasting health and wellness improvements.

In-network health care providers for both insurance plans are available to see patients at the wellness center.
The wellness center offers appointments for non-emergency treatment. Dial 911 or visit a nearby emergency room if you experience an emergency.
  • Appointments are strongly recommended for the best service to our patients. You can make an appointment by calling 678.377.4080 and following the prompts to choose the option for your healthcare provider.
  • Walk-ins may be seen if there is an available appointment. Every attempt will be made to see a walk-in patient, but those with scheduled appointments will be seen first. Walk-in will be offered at the next available appointment, but bear in mind there may not be same-day availability.
The last patient will be seen 15 minutes prior to closing time. Anyone who arrives within 15 minutes of closing time will be offered the next available appointment. 
If you are Medicare-eligible and covered under an active employee plan, you are eligible to use the wellness center and be seen by a provider.
  • Preventive care typically takes place when you’re feeling well. It usually involves quarterly, annual, or age specific exams and testing used for the prevention of illness.
  • Typically, acute/routine care typically is given when a patient is experiencing symptoms related to an episodic illness
We encourage using the Employee Wellness Center for primary care. If you choose to use the wellness center only for sick care, they will work with your primary care physician to improve your overall health.  
No. Labs taken during an appointment, such as during your annual physical, are included in the respective fee based on your medical plan.
It depends on the reason for the labs. If labs are preventive in nature, such as quarterly blood draws related to disease management, then labs are considered preventive and there is no charge. If a cancer screening is requested because of a history of cancer in one's family, those labs may incur the respective fee per your healthcare plan.
During scheduled work hours, employees will be granted up to two hours, as necessary, to visit the wellness center and return to work. Sick leave will be charged if an employee is away from the work site for more than two hours.

No. The administrator uses separate systems and servers. At no time will the County have access to individual patient information.
